Step 1: Check the 4 santol fruits are peeled and all seeds removed. Mince the edible fruit very finely by hand or with a food processor.
Step 2: Wrap the minced santol in a clean cheesecloth and wring firmly to remove its juice. Keep the squeezed fruit and set it aside; discard the extracted liquid.
Step 3: Heat the 3 tbsp oil in a pan. Sauté the crushed garlic (3 cloves) and minced red onion (1 medium) until the onion is translucent and the aromatics smell fragrant.
Step 4: Add the 1/4 lb ground pork to the aromatics. Break it up and cook until lightly browned, with no raw clumps; ground pork should reach 160 F (71 C).
Step 5: Add the squeezed minced santol and cook with the pork for 3–5 minutes, stirring.
Step 6: Pour in the 3 cups coconut cream and bring to a boil. Stir, then partially cover and maintain medium heat, adjusting to a steady simmer. Continue until the free liquid has reduced by about half, stirring to prevent sticking.
Step 7: Stir in the 3 tbsp shrimp paste, 2 sliced long green peppers and 5 Thai chilies. Cook uncovered, stirring regularly, until the peppers are cooked and the mixture is moist but has no pooling free liquid; stop before it scorches.
Step 8: Transfer the finished hot sinantolan to a serving dish.
Step 9: Share the sinantolan among four people and serve with ready-cooked rice alongside.
